will meet with you to develop a personalized home care plan and collaborate with the hospital discharge planner to coordinate all necessary home requirements.
Additionally, we will review both government and private insurance options to ensure you maximize all available benefits for at-home care.
To support continued recovery and well-being, Bayshore offers a wide range of services, including transportation, housekeeping, grocery and prescription pickup, personal support, medication reminders, meal preparation, nursing, dementia support, palliative and end-of-life care, as well as 24/7 services to provide assistance whenever it’s needed.
